WHAT DO I NEED TO PICK UP MY PRIZE?
You will need a driver's license, birth certificate, or another official government issued photo I.D. that has your mailing address displayed. In some specific cases, (i.e. trip winners) parents or legal guardians will be required in-person to sign a waiver form on the winner's behalf.

CAN SOMEONE ELSE PICK UP MY PRIZE?
YES - however... if you want someone else to pick up your prize, they must come with a signed letter from you and a photocopy of your official I.D. (driver's license, birth certificate or another official government issued I.D. that has your signature and address). Be sure to photocopy the side with your address! The letter must also name who will be picking up your prize and include their home telephone number. **Please note - this DOES NOT apply for Grand Prizes or lottery contests**

HOW LONG DO I HAVE TO PICK UP MY PRIZE?
30 days - We only hold prizes for 30 days unless your prize is something like concert tickets or event tickets which may expire sooner.

Dave FM Contest Rules & Regulations
1. The chances of winning depend on the total number of entrants who participate in the contest / call in to win for the contest.

2. After winning a prize, the winning individual and anyone within the same household and/or immediate family (father, mother, son/brother, sister/daughter) is not eligible to win any prize for 30 days from the date the prize was won.

3. The Contest is open to residents of Ontario, who are 18 years of age or older (unless stated otherwise) but excluding employees, directors and officers of CJDVFM, Corus Entertainment Inc. (“Corus”), any affiliate (as defined in the Canada Business Corporations Act) of CJDVFM or Corus (the “Affiliates”), employees, directors and officers of THE SPONSORS (the “Sponsor(s)”), employees, directors and officers of their respective advertising and promotional agencies, as well as family members of any of the foregoing.

4. All decisions of the judges are final.

5. Before being awarded the Prize, the Prize winner must correctly answer, unaided, a time-limited skill-testing mathematical question.

6. All entries become property of Dave FM - selected entrants who will be notified by e-mail or telephone. Entrants, by entering contests, consent to the use of their names and/or photo without compensation in any future publicity carried out by Dave FM in association with contests.

7. Dave FM, the sponsors and their agents are not responsible for technical, computer, telephone or production errors and may terminate or withdraw any contest found to contain such errors with without liability and without notice at their sole discretion.

8. Prizes may not be transferred, assigned or redeemed for cash.

9. Dave FM, the sponsors reserve the right to substitute prizes of equivalent kind and value.

10. Dave FM, the sponsors bear no responsibility for any expenses, personal injury, damage, pain or suffering whatsoever, arising out of or in connection with winning or using the prize or in any way taking part in a Dave FM contest.

11. Each winner must claim their prize within thirty (30) days of winning (the “Deadline”). If the Grand Prize winner fails either to claim the Prize or inform CJDVFM of his or her inability to claim the Prize before the Deadline, his or her entry will be forfeited and another eligible entrant will be selected.

12. Rules are subject to change without notice.

13. This Contest is void where prohibited by law and is subject to all federal, provincial, territorial and municipal laws and regulations of Canada and of each province, territory and municipality of Canada, respectively.
